Following the unexpected loss of all ten Minuteman I missiles assigned to Echo Flight on March 16, 1967, Strategic Air Command initiated an engineering investigation to determine the cause of the unprecedented system failure. The resulting reports confirmed that each missile entered a 'No-Go' condition within a very short period of time, requiring extensive troubleshooting before the launch facilities could be returned to operational status.
Investigators examined the Launch Control Center, guidance systems, communications equipment, and individual launch facilities. The final engineering assessment concluded that the most probable cause involved an electrical disturbance affecting the guidance and control systems. Although engineers identified components capable of producing similar failures, they were unable to fully reproduce the exact chain of events responsible for disabling all ten missiles almost simultaneously.
The report makes no reference to unidentified aerial phenomena, focusing exclusively on the technical aspects of the missile outage. However, because the engineering investigation confirms the timing and scale of the shutdown, it serves as one of the most important official documents associated with the Malmstrom incident. Researchers have frequently compared the report with witness testimony from launch officers and security personnel who described unusual aerial activity occurring at approximately the same time.
Today, the Echo Flight engineering report remains one of the strongest pieces of documentary evidence confirming that the missile shutdown itself was a genuine event investigated by the United States Air Force.
